Here is the template pharmacies should use to upload their patient information so it can be stored in the database.

Please inform pharmacies that this is the template that should be used to upload patient information. They should make a copy to save to their Google Drive and use this format when uploading patient information. Once this information is in the data base it will allow them to avoid manually typing in patient information when creating a new order. 
This information is used to pre-populate the system with their customer's logistics information so that the data pre-populates on the order entry screen.
